This release is optional. Saying no will not affect whether the participant can take part in Group Lessons.

What you allow

If you check the photo and video box, you allow Practicing Musician and the Host for your class to:

• Take photos, video and audio recordings of the participant during class

• Use any quotes or testimonials the participant chooses to give us

Where we may use them

• Our websites, social media accounts and email newsletters

• Printed flyers, posters and brochures

• Reports to Hosts and program partners

• Presentations to donors and funders, and grant applications

• News stories about the program

How we use names

• Adults: first name and last initial, unless you give us separate permission to use a full name.

• Participants under 18: first name only. We never pair a minor’s image with a school, address or other details that could identify them.

• We never publish anyone’s contact information.

What you should know

• You won’t be paid, and you won’t review photos or video before we use them.

• Practicing Musician owns the photos and video it takes. The Host owns the photos and video it takes, and each is responsible for its own use.

• We won’t use images in a way that is misleading or embarrassing.

Changing your mind

You can withdraw this permission at any time by emailing pmlistens@practicingmusician.com. We will stop new uses within [30 days] and remove the participant from our own website and social media where we reasonably can. We can’t recall printed materials already handed out, or posts other people have already shared.
